Personal information is any information that can be used to identify you or that can be linked to you.
I do not collect personal information through this website unless you chose to provide me with your contact details. If you do, I will keep your contact details secure in a password protected electronic database for no longer than is necessary. If you tell me you don’t want to pursue a course of therapy with me, I will remove your contact details from my records unless you give me your permission to retain them.
If you decide to see me for a course of therapy, I will keep your contact details, therapy notes and any other related material confidential and I comply with the General Data Protection Regulation.
I hold your information so that I can provide you with safe and effective therapy. I may also use your information so that I can check and review the quality of the care I provide. This helps me to improve my practice.
All electronic records are kept in a password protected electronic file. All paper records are kept in a locked filing cabinet unless I am transporting them to an external therapy room when they will be kept in a locked briefcase until they are required.
I will only share agreed information about your therapy, for example with your GP, if you provide your written consent. Exception to this is if I believe you are at imminent risk of harm. Sometimes I may also need to share your information without your consent so that other people, including healthcare staff, children or others with safeguarding needs, are protected from risk of harm. These circumstances are rare.
Information about confidentiality and sharing data will be made explicit in the consent to treatment information which you will be asked to sign prior to your first appointment.
